Symptoms

1)
After upgrading from OC/RDC 4.6.6 to 5.4:  some RDC Onsite users are unable to approve CRFs and getting the below error:

i.e.

 

2)
It was determined that the problem users do have the 'APPROVE' privilege granted along with the required roles and study/site access.
