Search By Selection Box Not Showing Up On Telephone Numbers Window

Symptoms
When opening the Telephone Numbers window, the "Search By" group box and its radio buttons that toggles searching between "Telephone Number" and "Geographic Area" are missing. Enabling the "Service Request > Product Service Request - PSR > Default Search to Telephone Numbers on TN Search" system preference to define the default search, changes the window to the proper type of search, but there is no ability for the user to modify the type of search dynamically.
