File Manager(FM) Startup Fails with 'Vault Error' and Uploading Attachments in Web Client Fails with Error:'HTTP Status 404' (Doc ID 2930592.1)

Symptoms

Uploading attachments fail with error.

ERROR
In Web Client: 'HTTP Status 404'

Startup script run from Cmd Prompt: 'Vault Error'

No error in FM logs.


STEPS
The issue can be reproduced at will with the following steps:
1.Log in to Web Client - http://<FQDN>:<port>/Agile/PLMServlet
2.Go to any object and click on Attachments tab
3.Click Add and then choose  radio button 'Use standard browser capabilities'
5.Click Ok
6.Select a file to upload and click Add
8.Upload fails with error : HTTP Status 404
