Symptoms

The com.portal.pfc.infranet.connpool.ConnectionPool quickly reaches maximum size and ceases to provide CM connections.
Via Mbean, it can be observed that the number of TotalConnections reaches max (e.g. 360) and the number of UsedConnections increases until it eventually reaches that of max and the pool becomes inoperable.
Stopping the traffic does not cause the number of used connections to drop and the pool does not re-balance to configured MinPoolSize nor UsedConnections number drop.
