Mapping with Same Configuration Behaves Differently Between Messaging Server Version 8.1.0.12 and 8.1.0.16

Symptoms
The rate limiting feature seems to have been broken when patching MMP systems from 8.1.0.12 to 8.1.0.16.
We have a metermaid callout in FROM_ACCESS to check if the limit has been reached and a metermaid callout in LOG_ACTION to increment the counter.
Overview of the scenario:
* FROM_ACCESS seems to no longer be doing the call to check the limit.
* LOG_ACTION probe format must have changed because the wrong values are being used.
On a test system, backed out from 8.1.0.16 to 8.1.0.12 and observe the following:
* FROM_ACCESS call happens again, with the correct value.
* LOG_ACTION call happens, with a different but still wrong value.
