ECM 812 Conditional Filtering Does Not Work With Case Creation Form

Symptoms
In ECM 804 we had type, subtype , subclass1 and subclass2 as dropdowns on create case page. Once user select the "type" the respective subtype, suclass1 and subclass2 shown as a dropdown. We are trying to achieve same functionality in 812 but it is not working.
Using below table for configuration :-
1) KDD_CASEATTRBT_MASTER
2) KDD_CASEATTRBT_MASTER_TL
3) KDD_CASEATTRBT_VAL
4) KDD_CASEATTRBT_BHVR
5) KDD_CASEATTRBT_COLMNID_MAP
