Not Able to Create Lookup and Static Table Drivers in PFT UI Doing Sanity Testing After New Installation

Symptoms
On PFT 8.1, while doing sanity testing in new instance, user is not able to create Lookup and Static Table Drivers in Profitability Mgmt -> Rule Specification.
In the Static Table Drivers screen, no common chart of account dimension members appear from the Member Search screen.
Without members selected, the save also fails with the following errors:

1029 Select Leaf or Nodes from Hierarchy Browser for the Key
1518 Please define Coefficients
In the Lookup Tables creation screen, both the Select Source table and Lookup table drop downs are blank.
