Symptoms

When user try to add attachments the following error occurs.

ORA-00001: unique constraint (AGILE.VERSION_PK) violated

The issue can be reproduced at will with the following steps:

Login to Agile Web Client - http(s)://<FQDN>:<port>/Agile/PLMServlet
Go to User profile --> Attachments
Add an attachment.
The above error is observed.
