Symptoms
The selections made by a user on the "Form Options" and "Request Settings" pages during request creation are not saved. when Users select their choice of setting for the request like Header & Footer Text, Page Size, Blank Form format, Prevent Content Extracts and Copying - the values get reset to default automatically, on the Review and Submit page. This results in the requests to be generated in unintended format when they are submitted. This behavior is consistent across all types of PDF requests.
