Misalignment/Incorrect Output Format Of Controls Displayed In Blank PDF Forms

Symptoms
It was observed that Blank PDF Requests shows misalignment or incorrect output format of Radio Controls for the forms which have the Text Control as well as Dropdown Control. This occur for questions that contained both text and dropdown controls, such as when collecting lab results (text) and units (dropdown).
For example, if a Form have 3 items with Group Controls (Text control and Dropdown control) then we would see the Dropdown controls displayed at the beginning of the page and then it will display all other items of the Form.
