Symptoms

Following error is thrown during pack installation:
[FATAL] 2024-10-15 15:11:08.597 [main] fatal - Error in Model upload. So aborting installation
[FATAL] 2024-10-15 15:11:08.598 [main] fatal - Data Model Failed...Check ModelUpload log for more details
[FATAL] 2024-10-15 15:11:08.598 [main] fatal - Error77: Error occured while performing DataModel Upload
  null
reverting default.properties
App installation failed for OFS_NGECM
Underlying error in Model Upload log:
ORA-01408: such column list already indexed
 ERROR WHILE FIRING INDEXES TO DATABASE
 Error Executing Alter Table Scripts.
