When attempting to add Affected Item in Change Order below error is observed

ORA-00001: Violate the condition that Must be a unique constraint (AGILE.REV_PK)

The issue can be reproduced at will with the following steps:

1. Login to Agile Web Client - http(s)://<FQDN>:<port>/Agile/PLMServlet
2. Create a change order object.
3. Create an Item.
4. Add a site to the item.
5. Go to the new created change object.
6. Add the new item into the Affected Item table of the created change object.
