Job Return -11 / Failed to Create Enough Semaphores Running Ledger Stat Allocation (Doc ID 359841.1)

In Oracle Performance Analyzer 4.5, Ledger Stat-based Allocations are receiving a Job Returned -11 error in the Server Status Update screen while the Request Queue (rq) log contains errors like the following:

Mon Feb 20 08:45:34 2006 : Job number 10517 started with process id 8498.
Mon Feb 20 08:45:34 2006 : Command: /d01/oracle/ofsa-4539/bin/ofspa
Mon Feb 20 08:46:35 2006 : Process 8498: died. (Signal 11) core dumped, saved in core.8498
Mon Feb 20 08:46:35 2006 : Process ID 8498: completed with return code -11.

Additionally, ofspa.xxxx files in the server directory /OFSA_Install/log contain the following message:

Running allocations with: 101208, 0, username, 09/30/2005, 0.
CMultileafLUT: Failed to create enough semaphores.
Error: (1023) Failed to create updater for summary table.

A valid unique index does exist on the LEDGER_STAT table.
