OGL EPF: Creation of New Time dimension Hierarchy Has Failed with ORA-01400

Symptoms

When running a Calendar Rule (Program - Enterprise Performance Foundation General Ledger Calendar Rule Engine) that brings period date information from Oracle General Ledger (OGL) into Enterprise Performance Foundation Administrator, the following error appears in the "View Log":

The creation of the new Time dimension hierarchy <Calendar Hierarchy Name> has failed.
Process registration has failed.
Process registration has failed.

As a result, no Calendar Periods are transferred from OGL.

If you turn on the FND: Debug system profile option and run the Calendar Rule, the following error is output to the table FND_LOG_MESSAGES:

ORA-01400: cannot insert NULL into ("FEM"."FEM_OBJECT_DEFINITION_B"."EFFECTIVE_START_DATE")
