Getting TNS-12545 and TNS-00515 in Sqlnet.Log Starting Request Queue (RQ)

Symptoms

You use an automated script to start the OFSA Request Queue (rq) and the rq is failing to start.  The following errors appear in a sqlnet.log file in the $OFSA_HOME/log directory:

Fatal NI connect error 12545, connecting to:
(DESCRIPTION=(CONNECT_DATA=(SID=*)(SERVICE_NAME=DATABASE.)(CID=(PROGRAM=)(HOST= server-name)(USER=ofsa)))(ADDRESS=(PROTOCOL=TCP)(HOST=DATABASE)(PORT=12345)))
