USER EXIT FETCHRDCIEXIT FAILED IN FUNCTION - CHECKSEQUENCE WHEN OPENING A PDF

Symptoms
Intermittent problem when opening a PDF page in the eCRF application, we have these error messages intermittently while the PDF is opening :
First message :
User Exit FETCHRDCIEXIT failed in Function - CheckSequence - error message follows
Second message :
Function called out of sequence
Third message :
Any open CRF will now be closed. All pending changes will be discarded.
After these 3 messages, you have to close RDC.
Although the error suggests that the problem is the sequence, all sequences seems to be well within the max limit (a lot less than the max value).
You can have this problem for a long period, then the problem disappears.
