When Deleting Unplanned Visits, Received Unexpected Unknown Error

Symptoms
On OC 4.5.1, when attempting to delete an Unplanned CRF in RDC in both PDF & Classic mode,
the following error occurs:
ERROR
Unplanned CRF cannot be entered for the current book."
and for PDF version, it displays the following after pressing OK.
"Unknown exception happened. exiting RDC"
The issue can be reproduced at will with the following steps:
1) Check the DCI Book in OC setup for the Whole Protocol, the Unplanned Use Allowed is set to "Y" under the DCI Constraint
2) Logon to RDC and for Patient 1106, Add unplanned CRF for DCI & make sure to approved the CRF as well. Approve the CRF
3) Login to OC and uncheck the Unplanned Use Allowed for DCI and save. exit
4) Login to RDC and access the same patient (1106) , make sure to Show Unplanned Pages
5) Click on the CRF Icon and delete the CRF.
6) The system will ask for confirmation of the CRF delete and when you press OK, it will delete the CRF and bring up the message
" Unplanned CRF cannot be entered for the current book."
after pressing OK.
"Unknown exception happened. exiting RDC"
