Menu Bar Missing From DCM and Data Entry After Dev6i Patchset 18 Upgrade (Doc ID 550803.1)

-- Problem Statement:
Before patchset 18 applied, you could navigate to Defintiion -> Dcm -> Dcm and the toolbar
with icons was listed under the Action, Move, Clear, Data, Query, Special, Help, Window bar.

The toolbar in question is the one with the icons:
save, exit, print, left arrow, right arrow, up arrow, down arrow, erase, insert, edit, query, list values, help.

In the instance where patchset 18 was applied, the toolbar with the icons was missing.

When you navigate to Data Entry (Data Entry, Initial login) and the toolbar is missing there as well.
