FRM-92101 ERROR AFTER INSTALL OF DISCOVERER ON ORACLE CLINICAL MID TIER

After installing Discoverer on the Oracle Clinical Middle Tier (as outlined in the AERs install guide) when you now try and log in to the Oracle Clinical application, the following error occurs:
FRM-92101: A failure has occurred on the Server.
You will need to reestablish your session.
The log in screen doesn't appear, the error message occurs before you enter your user name and password.
