RDC 4.5.3: Many 'XMLHttp Request Failed' Errors While Entering Data In DCI Forms (Doc ID 604386.1)

Symptoms

After a fresh RDC 4.5.3 installation, when users are trying data entry on new CRF and trying to bring up date field, it cannot be entered in the field. If user tries to enter the date manually, following error is displayed on the screen

XMLHttp request failed€™ and RDC closes the CRF.

Login on RDC 4.5.3 with URL to get debug information from RDC surround.

http://<serverName>/olsa/oc/rdcLogin.do?event=do s etup&debug=all

to generate debug information user follow these below steps:

  • Select study and patient, 
  • Click Open Casebooks for patient. 
  • Click on DCI form for patient. 
  • Form comes up fine in browser. 
After user entered comment in free text field.
  • Press Save icon to save form. 
  • Get Oracle RDC clinical Error on the screen:

€˜Dialog failed: ChangeReason


  • Trying to enter date in a date field. 

Get this error on the screen:

"This application has encountered a fatal error and will shut down.XMLHttp request failed: 9588289893 (Servlet error: Application Error.)".


RDC window with DCI form closes.
