CRF access intermittently throws error: This application has encountered a fatal error and will shut down. XMLHttp request failed: (Doc ID 763306.1)

Symptoms

PROBLEM DESCRIPTION: when attempting to open a CRF in HTML data entry with many discrepancies (for example 25) the system intermittently fails and presents the following error message:

ERROR MESSAGES:
“X Error
This application has encountered a fatal error and will shut down. XMLHttp request failed:
4662016703 (Servlet error: IO Exception: Unable to proceed further.).”


OCCURRENCE PATTERNS: intermittent, cannot be consistently replicated.

Steps to Reproduce:
 
1. Log-in to RDC 453 Onsite as superuser
2. Navigate to Casebooks page,
3. Select a Study / Site combination:
4. Click Go button under CRF search criteria
4. Open a CRF with specifications like the following:

     Study: A3191172
     Site: 1001
     Patient: 10011010
     Casebook: A3191172- (Cra Rdc Support – CRA role)

Form / Process Object Title:
RDC Onsite: Data Entry

CONFIGURATION MANAGEMENT DATA:

Applied following patches.
