Transformation Generates No Results in OFS_RPT_HIER Table in OFSA 4.5

Note: All OFSA 4.5 products have reached sustaining support.
In Oracle Risk Manager (RM) 4.5 or another OFSA module, when attempting to transform a Tree Rollup ID, no results are written into OFS_RPT_HIER_xxxxx table. The OFSA_PROCESS_ERRORS table contains no errors related to the Transformation ID.
Additionally, when attempting to open a Leaf Characteristics ID, the following error occurs:
OFSRM CAUSED A GENERAL PROTECTION FAULT IN MODULE BC450RTL.DLL AT 0001:8B1B
Steps to replicate:
- Log into Risk Manager
- Go to File > Open / New > Transformation ID
- Choose Source Type = Tree Rollup and select the appropriate Tree Rollup ID
- Run the Transformation ID
