Print CRF Truncates Right Side, Puts Footer On Separate Page & Print Extra Blank Page

This problem can occur on any platform.
Symptoms
On RDC 4.5.3 printing HTML data entry page, the margins of pages are
Shifted (the right side of CRF is not visualized and truncated - even for portrait pages) and a CRF of 1 page is Printed to 3 pages.
--Open a CRF in RDC 453 (means you have HTML Date Entry CRF)
--Click on print icon
--Send page to printer.
