Cannot Insert A New Member Through Member Management (Doc ID 801618.1)

Symptoms

On 7.2.0.1 in Production on SQL SERVER 2005:
When attempting to add a new item via Member Management,
the following error occurs.

ERROR
Error - SQLSTATE = 23000
Microsoft OLE DB Provider for SQL SERVER
Violation of PRIMARY KEY constraint 't_ep_item_pk'. Cannot insert suplicate key in object
'dbo.t_ep_item'. - Can not insert the new data into the table.

STEPS
The issue can be reproduced at will with the following steps:
1. Open Member Management
2. Add a new member at the lowest item level
3. Click Prod Sub Family as level in the left
4. Show members
5. Right click on the level to "Add Member"
6. Add member screen appears and all needed information is entered plus a unique member code
7. Select ok on the 'Add Member' screen and the below error is seen.


Error - SQLSTATE = 23000
