Generate Provisional Dci Form Fails (Doc ID 812003.1)

Symptoms

-- Problem Statement:
OC error 24311 - ORA-12203 when trying to generate a Provisional Layout

-- Steps To Reproduce:
1. Fresh install of OC RDC 4.5.3
2. Create DVGs/Questions/QG/DCMs/DCIs in that sequence.
3. Graphic layout generation completed.
4. After this:
Generate the DCI Form Version
Query for the created DCIs.
Place focus on the DCI row.
Select DCI Layout from the Special menu, then select Generate Provisional Layout.
Click Save. The system generates a provisional layout.
-> completes without any error.
Select Special, DCI Layout, and then Generate Provisional Dci form.
-> Gives an error (Screenshot attached) and teh end user loses connection to the application.

Can't complete smoketest

There is not database error in the trace files. There is not error in the alert.log either. The last statement in the sql trace file is:
