Switch Sheet Email Does Not Have the Switch Sheet Attachment

Information in this document applies to any platform.
During testing, when a planned switch sheet is submitted, the expected result is an email with subject "Request/Log # [number] is Requested" and an attached file containing the Header & Request tab. In this instance however, the email does not have an attachment.
swService log has these errors:
*ERROR* in SwEmailMgr::sendEmail
failed to convert plan 3106.18216 to XML format
