Filing Submission Fails Showing Error Message that 'The SERFF requirements have been changed' or 'The filing requirement was not found for sub_toi_id'

Tracker user prepares a SERFF filing and when submitted, it displays a message that submission can not be done as some requirements got changed. It posts a failed log entry stating the changes in the requirements. However sometime it can show another message that 'The Filing Requirement was not found for sub_toi_id'. In the failed activity, it also shows the same message.
