OGL EPF Balance Rule Code Combinations Not Properly Mapped -- No Incorrect GL Data Found

On 11i.FEM.D.1 Rollup #4 or R12, when rerunning the Dimension Rules using the 'Submit All Rules' button, the Dimension Rules do not fully populate the FEM_INTG_OGL_CCID_MAP table. The FEM_INTG_OGL_CCID_MAP table contains rows with -1 dimension values triggering errors when running the Balance Rules:


Loading standard balances

Loading average balances

The balances load cannot proceed because some code combinations of chart of accounts XYZ Chart of Accounts have not been mapped properly to the account-based dimensions. Please run the dimension rules for this chart of accounts to update the dimension mapping.

All Oracle General Ledger (GL) source data has already been checked for an incorrect data as per <Note: 394494.1> - OGL EPF: How to Troubleshoot the Balance Rule "Unmapped Accounts" Error.

The issue can only be resolved by first running the update statement before rerunning the Dimension Rules:

SQL> update fem.fem_intg_dim_rule_defs set max_ccid_processed = -1;

However, running the above statement triggers the Dimension Rules to run much longer. No error occurs when rerunning the Dimension Rules without the update statement.  The error does not reproduce at will and appears randomly for some periods.

Steps To Reproduce
The issue can be reproduced at will with the following steps:

1. Run Dimension Rules EPF- click the "Submit all Rules"
2. Run the Balance Rules
3. Error appears in Balance Rule logs
