Phone "ADD" Drop Down On Account Creation Locks

When creating an account (Consumer or Business), the New Phone Number Type drop down box often freezes and you cannot use your mouse to select a type (Home/Work/etc.) when Adding the phone number. Note that you can still use your keyboard up/down arrows to move through the list and select, but mouse clicking will not work.
-- Steps To Reproduce:
Open Account Creation wizard, click on Phone "Add" button; and select a phone other than Home. Then Cancel the New Phone Number dialogue box. (This time, everything is fine)
Cancel the Wizard and start another AC Wizard; click on phone "Add" button; this time, you can NOT use mouse to select any phone types, but only hear a "dong" sound.
