RDC ONSITE 4.6: DATA ENTRY WINDOW, HTML CODES APPEAR ON CRF PAGES

Symptoms
On : 4.6.0 version, Data-Entry, Log-In
While trying to enter data in RDC, the HTML Codes appear on CRF pages behind the fields defined for the CRF. This made it impossible to enter data.
EXPECTED BEHAVIOR

Expect to enter data without the HTML Codes appearing.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Login to RDC Onsite.
2. Select a study & site.
3. Select patient & Open Casebook and press Go.
4. Open the CRF icon to enter data
The HTML Codes appear behind the defined fields for the CRF.
