Fusion Applications: Corrupted Role Template Prevent Data Role Generation in Authorization Policy Manager (APM)

In Authorization Policy Manager (APM) console, when you search and open a data role template, you will get an error
APM-10082: The following list of external-role(s) defined in the current template were not found in the identity store: <base role 1> <base role 2> <base role 3>

If you click OK in the message window, then hit Generate Roles button, either nothing happens or you will get an error
APM-10009:Template operation did not complete due to system error. Please contact administrator for detail.

If you select a role under Inconsistently Deleted Roles section, and click Recreate Role, you will get an error

APM-10026: An error occurred while trying to perform role reconciliation. The most likely issue is that some artifacts (ie. External role, dimension value) was changed. Please reload the document and re-execute the template, or contact your System Administrator.
