On : 11.1.4.0.0 version, Analyze Workforce Deployment-Evaluate Performance

ACTUAL BEHAVIOR
---------------
When attempting to run Manage Eligibility Batch Process the list of performance documents contains multiple redundant / duplicate lines.

EXPECTED BEHAVIOR
-----------------------
When running Manage Eligibility Batch Process the list of performance documents should contain unique names.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Setup and Maintenance > Tasks > Manage Eligibility Batch Process
2. Enter Effective as of Date
3. Enter Document Type
4. Enter Performance Document Start Date
5. Enter Performance Document End Date
6. Click on Performance Document Name LOV
7. Multiple redundant forms appear on LOV

BUSINESS IMPACT

The issue has the following business impact:
Due to this issue, users cannot begin performance review on certain employees - templates are not associating/available. Review at stand-still.
