SSO - Users Unable to Access Zone Following Zone Upgrade or Restart

Information in this document applies to any platform.
After a SSO-enabled zone upgrade or restart, some users are unable to access the zone even though their accounts are setup correctly and were working fine prior to the upgrade.
If affected users access the zone directly and log in, then subsequent access via SSO is successful.
One of the symptoms of this issue is "ErrorReason=92". This might show up in a log (Taleo-side) and/or an error screen seen by user, depending upon specifics of configuration.
An SSO-enabled zone is upgraded.
