CREATE APPOINTMENT RELATED TO AN OPPTY: VISIBILITY NOT CORRECT

When creating an appointment the 'Related To' field when clicked on and a opportunity selected the search box pop up on the top part of the page instead of next to the 'Related To ' field.
Expect the opportunity pop up search box be next to the 'Related To ' field.
The issue can be reproduced at will with the following steps:
2.Manage Customer page - Select Calendar icon at the top left hand corner
3.Create appointment ,
4.On the Appointment page select the field " Relate To"
5.Choose Opportunity from the drop down list
