Approve Reject Not Working Via Email

Fusion Applications version 11.1.8 is been used.
Problem is that users are unable to approve transactions via email
Steps to reproduce the Behavior
1. Submit a new goal
2. Manager receives email and clicks on approve button
Transaction is not approved
