A Qualification Questionnaire Is Not Available Error When Accessing Qualification Subtab In Simplified UI

After the Release 9 upgrade, users are getting following message when clicking on the Qualification subtab on the Lead details page in Simplified UI.
"A qualification questionnaire is not available because a template was not identified for the Lead Qualification Template profile."
1. Login to Simplified UI
2. Navigate to Leads application
3. Create a new lead, save and continue
4. Click on the Qualification subtab
