Unable To View File Attachments At Header Or Line Levels For Approval Email Notifications (Doc ID 2001274.1)

Symptoms

Users are unable to see attachments at the  header or line level in approval email notifications. Users create a requisition and attach files (File Type = To Approver) at the Header Level and Line Level.
When the approval email/ FYI notification is received, one can not see these attachments in the email. These attachments are available in the Approval Form when accessed from the tasklist of the approver.
Also , one file flows through and is available on the email. If this file is attached irrespective of any number of other files attached, this is the only file available on the approval email notification.
 
User's expect all file attachments to appear within the resulting approvals e-mail notification.

The issue can be reproduced by performing the following steps:

Attach two files to the requisition; a ppt and Excel file.
