Error Message Upon Saving Lead Qualification (Doc ID 2008543.1)

Symptoms

On : 11.1.9.2.0 version, Lead Management


New Lead Qualification Template has been set up and enabled using Manage Sales Lead Administrator Profile Values. But there is an error when a user tries to save the Lead Qualification Template in Simplified UI - This issue does not happen with all the uses though.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Create template using task Manage Sales Lead Assessment Template
2. Deploy template
3. Enable assessment by going to Setup & Maintenance -> Manage Sales Lead Administrator Profile Values -> add a row for Site level and new template within these three profile options:
MKL_LEAD_ASSESSMENT_TEMPLATE - New Test Template
MKL_LEAD_QUALIFICATION_TEMPLATE - New Test Template
MKL_PARTNER_LEAD_QUALIFICATION_TEMPLATE - New Test Template
4. Navigate to Simplified UI
5. Click Leads icon
6. Open existing lead or create new lead
7. Click Lead Qualification icon
8. Answer questions within the lead assessment template
9. Click Save button (this is when I receive the error message every time, despite the answers to the questions or which lead I'm working with)
