Not Able to Connect to the DBaaS Instance via SSH due Authentication Token Manipulation Error or Under Java Cloud Service Provisioning

Symptoms
There are two scenarios where connectivity to Database as a Service (DBaaS) does not succeed.
1. When trying to access to the DBaaS VM via SSH as the user these messages are displayed.
$ ssh -i <path-to-ssh-private-key> user@<vm-public-ip> Warning: untrusted X11 forwarding setup failed: xauth key data not generated Warning: No xauth data; using fake authentication data for X11 forwarding. You are required to change your password immediately (password aged) Authorized uses only. All activity may be monitored and reported. WARNING: Your password has expired. You must change your password now and login again! Changing password for user. Changing password for user. (current) UNIX password: passwd: Authentication token manipulation error Connection to XX.XX.XX.XX closed.
2. On Java Cloud Service (JCS) instance provisioning following error messages are observed. Working DB credentials were delivered.
