Attachment Field Is Disabled In Assessment Page

Symptoms

On : 11.1.7.0.0 version, Manage Partners - Manage Partner Accounts

ACTUAL BEHAVIOR
---------------
Attachment field is disabled in Partner Assessment page

Partner>>Assessment Node>>Assessment Template Selected for assess>>Now here below Template Name attachment field is there which is disabled.
 We need to get enable same button and also we need attachment feature against each question so user will be able to attach documents against each questions.

EXPECTED BEHAVIOR

User expects to be able to upload attachments in that screen

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
Replicate the issue in CHA

Navigate to Navigator ->Partners
Select or create a new partner record
Drill down on Partner Name
Click on Assessments
Create a new record
Select one available assessment Template ( if not available one needs to be defined)
In this screen you can see the label 'Attachment' but the application does not allow to select one
