Custom Business Flow Does Not Work (Doc ID 2022924.1)

Custom Business Flow is not working. Found the custom Approval flow is not working with any users.
The initial action taken by BPM component is not working, so not sure whether the BP flow is kicked or not.

Expect that the business flow kicks and field updates happen as per the workflow definition

The issue can be reproduced at will with the following steps:
A. Create a custom workflow with details as

1. User > on Opportunity > Update "Sales Stage" to "ST0" >
2. Object Workflow > BusinessProcess_ST0_Approval on Opportunity > Kicks BPFlow "Opportunity_ST0_Approval"
3. BP Flow > Opportunity_ST0_Approval >
  Opportunity > Update "Approval Status" to "APPROVALPENDING"
  NA > Approve in Houman Task
  Opportunity > Update "Approval Status" to "APPROVALPENDING"
4. Before Update trigger = BU_UpdateSalesStagebasedonApprovalS
  Sequence = Update "LastNumber_c"
  Opportunity = Update "Registration_c"
  Opportunity = Update "Sales Stage" to "ST1"

B. The action#3 does not take place.
The println() embedded in the condition script of OWF in #2 is generated.

C. 1. [User] update "Sales Stage" to "ST0"
2. [Object Workflow]
 The println() text is generated which is embedded in the conditional script for the OWF.
3.[ BPM]
  Approval Status is not generated even after 5 min.
  Human Task is not assigned to Approver even after 5min.

The issue has the following business impact:
Due to this issue, users cannot have the custom workflow code working
