Lead Qualification Template Not Shown In Simplified UI

On : version, Leads, Sales Campaigns

While editing Lead, In Desktop UI we can see the template set for Lead Qualification but in Simplified UI is displayed: A qualification questionnaire is not available because a template was not identified for the Lead Qualification Template profile.

While editing Lead, users should see the Site-level questionnaire both in Desktop UI and in Simplified UI.

The issue can be reproduced at will with the following steps:
1. Open Lead
2. Navigate to Qualification Tab
