Mobile Approval Contains Incorrect "From" Address and is Missing the Token in the Email Subject

Symptoms

User received an Mobile Approval email but the "From" field is not from "mobileapproval.bigmachines.com".
Due to this issue, user reply to approve a quote will not work. The token is also missing from Subject line causing a reply to the mobile approval address to fail.
