Participant Feedback has inconsistent/missing data

During UAT workshops we noticed that the performance questionnaires appear to have different formatting and a missing section when viewing the questionnaire through the Feedback Participant viewpoint as compared to when the manager goes in to preview the questionnaire information.

Participant should also able to see PAREXEL section

The issue can be reproduced at will with the following steps:
1. Navigator > My Information > Performance > Click My Feedback Requests tab > Click 'Begin' button > Notice that the PAREXEL Values section does not appear.
