Field Merges: No Field Merge Appears when Adding Custom Contact Field to Form

Symptoms
When adding a custom contact field to a form, no field merge code appears in the field like it does when adding standard contact fields.
In the example below, the standard contact fields (Email Address, First Name, Last Name, Company) show the yellow-highlighted text which indicates a field merge code. The custom contact field (Sample Contact Field) does not.
