Unable to Access CPQ Cloud from Salesforce after CPQ Cloud Refresh (Doc ID 2095988.1)

Symptoms

After a refresh from one CPQ environment to another, users are unable to successfully access CPQ via Salesforce (SFDC).

They are taken to a blank screen when attempting to create or edit a quote.

OAuth is enabled on the site and version 5.0 of the managed package is being used.

No error messages can be observed.
