FC0090 Error When Doing Synchronization

Why users receive ERROR FC0090 when doing synchronization from Outlook plugin after an P2T?
Exception Description: A type could not be found for interface class [oracle.jbo.service.errors.ServiceErrorMessage]. Please make sure that the type has been defined. In addition, the interface classloader should be a member of the helper context classloader hierarchy: it appears as if this is [true].
Also, it seems that the Application Duty Role has lost Duty hierarchy.
