Error When Clicking the Submit Action After Upgrading to 2015 R2 [Unknown Error]

There is unknown error showing up after click submit action in a quote after the site upgraded to 2015 R2. The error message is listed below:
An unknown error (class com.bm.xchange.util.BMSystemException) has occurred. Please contact your system administrator.
To help us track this error, please note the date and time in your report: 2016-01-05 15:39:59.
This issue only happens if the Required in Sequence is checked on the first level of approval reasons of the submit action.
The site is upgraded to 2015 R2.
