ACTUAL BEHAVIOR
---------------
Unable to change the Date Format on Add Forecast Item page in Simplified UI.

EXPECTED BEHAVIOR
-----------------------
When setting the "Date Format" under "Regional Settings" under "Set Preferences" under "Settings and Actions" (under user dropdown in upper right-hand corner of Simplified UI), it should use the corresponding date format on the Add Forecast Items page. Verified the expected behavior in other pages like Create Opportunity, Create Appointment, and Create Task.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1) In Simplified UI, under "Settings and Action" (under user dropdown in upper right-hand corner) select "Set Preferences".
2) Under Regional Settings, set the Date Format.
3) On the Create Opportunity page, the date format is used as specified in step #2.
4) On the Create Forecast Items page, the date format is not used as specified in step #2.

Due to this issue, users cannot see the date in the expected format.
