Opportunity Contact Association IS Going Unsuccessful

On : version, Opportunities

When attempting to associate contacts to multiple opportunities:
the following error occurs.

Import status shows Unsuccessful

The issue can be reproduced at will with the following steps:
1. Create data file for import with column headers as mentioned below:

Optynumber,Optynumber1,Contact_Os,Contact Registry ID / Contact OSR,Contact_Os1,Contact Registry ID / Contact OSR1,REL TYPE,REL DATE,REL TYPE1,REL DATE1

2. Run the import
3. The errors are displayed

The issue has the following business impact:
Due to this issue, it is not possible do perform Opportunity Contact association through import process.
